Apply for a Ground Transportation Permit

Ground Transportation Rules & Regulations

STEP 1: Acceptance
This document outlines the terms, conditions, restrictions, and obligations of the Permit. You must print this document, sign page #27, and submit it with your application. NOTE: You need only to submit page #27. Keep the rest of the document for your personal records.

STEP 2: Permit Application
Print and complete the appropriate permit application from the following list. For more information concerning the categories below, please refer to the Ground Transportation Rules & Regulations.
General Permit - Buses, Taxis, Door-to-door Shuttles, etc.
Limousine Permit
Off-Airport Parking Permit
Off-Airport Rental Car Permit

STEP 3: Submission
The application and permit must be executed by an authorized company representative. Additionally, to ensure prompt processing, please ensure that all insurance certificates with proper endorsements are included (Insurance Requirements). Complete applications should be mailed to:
